Need to apply for a new passport, renew your current one or add additional visa pages? Each of these tasks requires a different form from the U.S. State Department, and figuring out which one to use can be a challenge. To make your life easier, we've gathered all of the most commonly used forms right here -- along with handy checklists of supplementary materials to include with each application.
You can download the forms you need by clicking on the name of the form to go to the U.S. State Department's website; there you'll find instructions on how to get each form and who should use it. You can use the lists below to check off each part of your application as you finish it. The lists can be printed for reference, or you can simply check off each box onscreen.

Form DS-11 One 2x2 inch color photo, taken within the past six months
Proof of U.S. citizenship or nationality (such as a certified birth certificate)
Proof of identity (such as a driver's license) and a photocopy of that ID
Application fee ($145)
Form DS-11Form DS-3053 (necessary if only one parent is appearing with the child at time of application)
One 2x2 inch color photo, taken within the past six months
Proof of child's U.S. citizenship (such as a certified birth certificate)
Proof of adult's relationship to child (birth certificate may suffice)
Proof of adult's identity (such as a passport or driver's license) and a photocopy of that ID

Form DS-82 Existing U.S. passport
One 2x2 inch color photo, taken within the past six months
Renewal fee ($110)
Form DS-11Form DS-3053 (necessary if only one parent is appearing with the child at time of application)
One 2x2 inch color photo, taken within the past six months
Proof of child's U.S. citizenship (such as a certified birth certificate)
Proof of adult's relationship to child (birth certificate may suffice)
Proof of adult's identity (such as a passport or driver's license) and a photocopy of that ID

Form DS-82 (if you previously had a regular passport and are applying for the passport card as a renewal)

Form DS-5504 (if less than one year has passed since your passport was issued) OR
Form DS-82 (if more than a year has passed since your passport was issued)

Certified copy of document specifying name change
One 2x2 inch color photo, taken within the past six months
Renewal fee ($110) if you've had your passport longer than one year or if your name was changed for any reason other than marriage or court order
Form DS-11Form DS-64 One 2x2 inch color photo, taken within the past six months
Proof of U.S. citizenship or nationality (such as a certified birth certificate)
Proof of identity (such as a driver's license) and a photocopy of that ID
Application fee ($145 for ages 16 and up, $115 for children under 16)
